Admission to Saint Francis Home usually begins with a visit.  We encourage you to schedule a tour, so you can see for yourself the attention to details that contribute so much to the quality of life in an assisted care environment.  Whenever possible, we like to meet with prospective new residents, introduce them to some of the staff, and to their fellow residents, so they can gain an appreciation for the sense of community that makes Saint Francis Home unique.

We encourage you to come for a visit and tour, during which, you can have your questions answered and get a feel for the facility.

If you would like to streamline the process, we encourage you to download our disclosure statement and preadmission form. These documents provide the foundation upon which to build our dialogue concerning your needs and the services provided at Saint Francis Home.  If you fill out the form in advance of your visit, and either bring it with you or mail it ahead of time, we’ll have a head-start. Approved residential applicants must also submit to a medical examination no more than 30 days prior to admission. The Report of Resident Physical Examination is included with the pre-admission form. You may also download that form above.
